The 2026 National Truck Accident Research Centre major incident report gives Australian truck operators a useful, if challenging, snapshot of where serious heavy vehicle risk is changing.
Released in October 2026 and based on de-identified NTI-insured heavy vehicle major-loss claims, the report points to progress on distraction and fatigue, while also highlighting renewed concern about inappropriate speed and following distance.

The headline improvement is significant. Major heavy vehicle incidents linked to inattention or distraction fell in 2025, and fatigue-related major incident rates also reduced sharply. For operators investing in driver coaching, fatigue systems, telematics, route planning and in-cab technology, that is a reminder that risk controls can matter. However, the report does not prove that any single intervention caused the improvement, and it is not a complete count of every truck crash in Australia.

For insurance purposes, the more important message is that claim patterns remain heavily influenced by human factors. NTARC reported that human factors accounted for more than half of the major incidents examined for 2025. Inattention or distraction, inappropriate speed and inadequate following distance together remained the leading group of human-factor contributors, even as some individual categories moved in the right direction.

Inappropriate speed deserves close attention because the term is broader than simply exceeding a posted limit. It can mean travelling too fast for the road layout, weather, traffic, vehicle mass, load configuration or descent conditions. That distinction matters after an incident because a claim review may look beyond whether the truck was technically under the speed limit. Maintenance records, driver instructions, journey planning, load details and dashcam or telematics data can all become part of the evidence trail.

Following distance is another practical issue for fleets, especially rigid trucks working in metropolitan and suburban traffic. Rear-end and same-direction crashes can create vehicle damage, third-party property exposure, cargo disruption and downtime in one event. Operators reviewing truck insurance premiums should therefore look at the operational causes behind incidents, not just the final claim cost.

Useful review points include:

  • whether speed policies account for weather, gradients, mass and load stability;
  • how drivers are coached after harsh braking, close-following or lane-departure events;
  • whether delivery schedules create pressure to continue in unsuitable conditions;
  • how quickly incident details, photos and telematics are preserved for truck insurance claims;
  • whether subcontractors and casual drivers receive the same safety expectations as employed drivers.

The report is not a reason to assume insurance outcomes will automatically improve for every operator. It is, however, a timely prompt to connect safety management with insurance preparation. Fleets that can show consistent systems, documented driver management and clear post-incident evidence are usually in a stronger position to explain their risk profile than those relying on informal practices after something has gone wrong.
